Established in 1979, Smoky Hill Foundation For Chemical Dependency, Inc-Outpatient Treatment offers outpatient treatment services for the prevention, intervention and treatment of substance abuse. Deb Tremblay is a licensed clinical social worker and licensed clinical addictions counselor with more than 20 years of professional work experience. She has experience helping patents with motivation, self-esteem and effectively coping with life changes and addictions. She believes that you are the expert of your story and you have many strengths that will assist you in overcoming obstacles that challenge you.
Michelle Ghumm is a Kansas-certified peer mentor with Smoky Hill Foundation. She has been in long-term substance abuse recovery for 6+ years. She has a Bachelor's Degree in Justice Studies from FHSU. She has also completed the training and hours necessary through KDADS to become a certified-peer mentor. Her passion is offering hope and sharing her experiences in long-term recovery. Brandi Zimmerman attended Fort Hays State University where she received her Bachelor of Social Work degree in May 2021. She then continued her education at Fort Hays State University where she got her Master of Social Work degree in May 2022. She has been with Smoky Hill Foundation since June 2021. She has worked in the Hays and Colby offices. She conducts individual sessions, group sessions and evaluations.
Kasey Grauerholz is the addiction counselor for our Phillipsburg Outreach office. She is a licensed addiction counselor and licensed bachelor social worker. Kasey graduated from Fort Hays State University in 2018. Kasey began her career in child welfare for four years, then joined Smoky Hill Foundation in 2020. In her free time, Kasey enjoys camping, reading, doing DIY projects and spending time with her family.
Carmen Mulkey is a case worker at Smoky Hill Foundation. She graduated from Fort Hays State University with her Bachelor’s in Social Work in 2017. Carmen is a licensed social worker and is currently working on obtaining a license in addictions counseling. She enjoys traveling, kayaking, reading and spending time with friends and family.
Jobeth Haselhorst is the Treatment Director of Smoky Hill Foundation for Chemical Dependency, Inc. She is a Licensed Master Social Worker and a Licensed Master Addiction Counselor. She graduated from Fort Hays State University with a Bachelor of Arts in Sociology with a certificate in Life Stages and Transitions in 2010 and a Bachelor of Arts in Social Work in 2017, with a Master of Arts in Social Work degree from the University of Kansas in 2018. Prior to becoming Treatment Director in January 2020, she began her career with Smoky Hill Foundation as a Chronic Disease Risk Reduction Coordinator in 2014.
